ABILITY TO WORK

This questionnaire is primarily designed for work related injury or disability, and asks the question; how impeded is someone’s ability to work as a result of their problem. This questionnaire should often be used in unison with the “Basic Daily Living” questionnaire.

Instead of asking for regularity and severity the “Ability To Work” questionnaire asks “Can you perform the activity” and “Discomfort during and after the activity”. The resulting severity scoring system remains the same – the bigger the number the worse the scenario. In terms of documenting improvement and clinical justification it nowadays appears very important to demonstrate that someone’s daily function is either being improved or at least maintained at an acceptable level.

ADHD

Attention-Deficit Hyperactivity Disorder (ADHD) is a neurobehavioral developmental disorder affecting about 3-5% of the world's population. It typically presents during childhood, and is characterized by a persistent pattern of impulsiveness and inattention, with or without a component of hyperactivity. ADHD occurs twice as commonly in boys as in girls. ADHD is generally a chronic disorder with 5 to 12% of individuals diagnosed in childhood continuing to meet diagnostic criteria in adulthood. As they mature, adolescents and adults with ADHD are likely to develop coping mechanisms to compensate for their impairment.

This is the original questionnaire that led onto all the rest of these tools. It is based on the DSM-IV classification of ADHD. The reason I first developed this questionnaire was an observation that many people and even some health professionals were using the DSM-IV as a check-list to diagnose ADHD – however it is made clear in the classification that diagnosis requires more stringent use of the criteria. The benefit of the regularity and severity columns that I developed were to help to differentiate kids who had severe symptoms, but only occasionally; versus kids who had mild symptoms all the time; versus kid’s who actually did demonstrate significant symptoms all the time – there are large clinical and lifestyle differences between each scenario.

ALLERGIES

Allergy is a disorder of the immune system often also referred to as atopy. Allergic reactions occur to environmental substances known as allergens. Common allergic reactions include eczema, hives, hay fever, asthma, food allergies, and reactions to the venom of stinging insects such as wasps and bees.

This questionnaire investigates classical atopic symptoms/signs, so it may identify a person that does have generalised allergic tendency or if specific items show up as scoring more highly; it may direct you toward more specific allergy. The questionnaire also determines if the person is aware of any trigger or aggravating factors. Some people will be oblivious to these factors if they haven’t previously investigated their possibility: So if none of these factors are ticked, it may indicate that the person will need to be guided through some sort of trigger identification process.

ANXIETY

Anxiety is a physiological and psychological state characterized by cognitive, somatic, emotional, and behavioural components. These components combine to create the painful feelings that are typically recognized as uneasiness, apprehension, or worry. Anxiety is a normal reaction to stress. It may help a person to deal with a difficult situation, for example at work or at school, by prompting one to cope with it. When anxiety becomes excessive, it may fall under the classification of an anxiety disorder.

This questionnaire should often be used in conjunction with the depression tool as they are very separate and distinct, but often co-existing phenomenon.

These questionnaires can be a very useful additional tool if at the end of an initial stage of care progress appears inadequate – this will help to identify emotional components to the problem/s. Often, just the identification of one or both of these issues will be enough to break the pattern and motivate the person to confront these issues, and, will greatly assist their program.

ARTHRITIS

Arthritis is a group of conditions involving damage to the joints of the body. Arthritis is the leading cause of disability in people older than fifty-five years. There are different forms of arthritis; each has a different cause.

This questionnaire has been designed to cover degenerative and inflammatory arthritides. If you review the pattern of the answers you should be able to determine – how generalised the complaint is, and whether it demonstrates more of a degenerative or inflammatory cycle. There are also some questions built in to suggest an allergy/immune involvement.

ASPERGER'S SYNDROME

Asperger syndrome is the autism spectrum disorder (ASD) in which there is no general delay in language or cognitive development. Like the more severe ASDs, it is characterized by difficulties in social interaction and restricted, stereotyped patterns of behaviour and interests.

This questionnaire covers the classic manifestations of Asperger’s syndrome. You will discover that a large group of people are incorrectly diagnosed with this condition, as the assumption is often that kid’s showing more severe forms of ADHD are Asperger’s. As you will see from the questions they are quite separate and distinct disorders behaviourally. Also beware if you have been diagnosed with both ADHD and Asperger’s – It is more useful clinically to differentiate which is the strongest disorder being manifest.

If the client is a child, the parent should complete the questionnaire, and also request that a teacher or carer completes the tool.

ASTHMA

Asthma is a chronic condition involving the respiratory system in which the airways occasionally constrict, become inflamed, and are lined with excessive amounts of mucus, often in response to one or more triggers. These episodes may be triggered by such things as exposure to an environmental stimulant such as an allergen, environmental tobacco smoke, cold or warm air, perfume, pet dander, moist air, exercise or exertion, or emotional stress. In children, the most common triggers are viral illnesses such as those that cause the common cold. This airway narrowing causes symptoms such as wheezing, shortness of breath, chest tightness, and coughing.

This questionnaire covers the most common signs, symptoms and effects of asthma. It includes some important questions to help screen for related spinal dysfunction, and some questions to suggest an immune system component.

AUTISM

Autism is a brain development disorder that is characterized by impaired social interaction and communication, and restricted and repetitive behaviour, all starting before a child is three years old. This set of signs distinguishes autism from milder autism spectrum disorders.
This questionnaire covers the most common signs, symptoms and effects of autism. This tool has thirty questions instead of the default twenty five.

BABY / INFANT HEALTH

This is a general health questionnaire for infants – the parent should complete the questionnaire.

BASIC DAILY LIVING

This questionnaire is primarily designed for work related injury or physical disabilities, and asks the question; how impeded is someone’s basic daily living functions as a result of their problem. This questionnaire should often be used in unison with the “Ability To Work” questionnaire. Instead of asking for regularity and severity the “Basic Daily Living” questionnaire asks “Can you perform the activity” and “Discomfort during and after the activity”. In terms of documenting improvement and clinical justification it nowadays appears very important to demonstrate that someone’s daily function is either being improved or at least maintained at an acceptable level.

In the cases of chronic work or accident related injury I would also recommend using a relevant body-part questionnaire as well as the anxiety and depression tools. This may sound like a lot of questionnaires but I explain this in two ways:

  1. Someone else is paying for the treatment so you are responsible to demonstrate to them that your program is justified.
  2. If the person wises to continue to receive care then they need to have evidence that it should be continued in the event of independent reviews and third party attempts to cease compensation.

CARPAL TUNNEL

Carpal Tunnel Syndrome (CTS) or median neuropathy at the wrist is a condition in which the median nerve is compressed at the wrist, leading to pain, tingling, and muscle weakness in the hand. True carpal tunnel only elicits symptoms in the thumb, index, and middle fingers, along the median nerve distribution, but some people may experience symptoms in the palm as well.

This questionnaire covers the most common signs, symptoms and effects of carpal tunnel syndrome. It includes some important questions to help screen for related spinal dysfunction, and some questions to suggest neurological compromise from a more central or internal source.

CEREBRAL PALSY

Cerebral palsy (CP) is an umbrella term encompassing a group of non-progressive, non-contagious conditions that cause physical disability in human development. Cerebral refers to the cerebrum, which is the affected area of the brain (although the disorder most likely involves connections between the cortex and other parts of the brain such as the cerebellum), and palsy refers to disorder of movement. CP is caused by damage to the motor control centres of the young developing brain and can occur during pregnancy (about 75%), during childbirth (about 5%) or after birth (about 15%) up to about age three.

This questionnaire covers the most common signs, symptoms and effects of Cerebral Palsy. This tool has thirty two questions instead of the default twenty five.

CHILD HEALTH / PAEDIATRIC

This is a general health questionnaire for children, primarily school age –the parent should complete the questionnaire unless the child is old and responsible enough.

CHRONIC FATIGUE / CFS

Chronic Fatigue Syndrome (CFS) is the most common name given to a poorly understood, variably debilitating disorder or disorders of uncertain causation. Symptoms of CFS include widespread muscle and joint pain, cognitive difficulties, chronic, often severe mental and physical exhaustion and other characteristic symptoms in a previously healthy and active person.

This questionnaire covers the most common signs, symptoms and effects of Chronic Fatigue Syndrome.

DEPRESSION

Depression refers to both expected and pathologically chronic or severe levels of sadness, perceived helplessness, disinterest, and other related emotions and behaviours.

This questionnaire should often be used in conjunction with the anxiety tool as they are very separate and distinct, but often co-existing phenomenon.

DIGESTIVE SYSTEM

This questionnaire covers common signs, symptoms and effects of digestive system dysfunction.

EPILEPSY

Epilepsy is a common chronic neurological disorder that is characterized by recurrent unprovoked seizures. These seizures are transient signs and/or symptoms due to abnormal, excessive or synchronous neuronal activity in the brain.

FERTILITY

Infertility primarily refers to the biological inability of a man or a woman to contribute to conception. Infertility may also refer to the state of a woman who is unable to carry a pregnancy to full term.

This questionnaire covers common signs, symptoms and effects associated with infertility. Since the intended outcome of a fertility care program is very easy to measure – successful conception and then birth; this questionnaire does not use a rating system – instead it is just a tick the box tool, to assist in taking an adequate and relevant history. It has forty four questions instead of the default twenty five. Depending on the answers given further consultation and/or investigation may be indicated.

FOOT / ANKLE

This questionnaire covers common signs, symptoms and effects of foot and ankle problems.
